What is a maintenance work order
The work order is the basic work unit of maintenance: every relevant service is born, executed and closed within an order. It plays three roles at the same time: it authorizes the work (with defined priority and approval), guides the execution (what to do, on which equipment, with which materials) and records what happened (cause, action, times and labor hours).
It is this third role that separates managed maintenance from maintenance that simply happens. Without a well-filled work order there is no history per equipment, and without history there is no reliable indicator, as we show in the article on maintenance work order management.
The fields that cannot be missing from a work order
The templates on this page were designed backwards: we started from the indicators that maintenance planning needs to calculate and defined the fields that feed them. A complete work order needs to contain:
- Work order number: unique identifier, the basis of any control and traceability.
- Equipment and tag: the address of the service; it is what accumulates history per asset.
- Location or area: where the equipment sits in the plant.
- Maintenance type: corrective, preventive, predictive, inspection or improvement; it supports the analysis of the order profile.
- Priority: emergency, high, medium or low; it disciplines the scheduling queue.
- Problem or service description: the observed symptom or the planned task.
- Identified failure cause: the field that turns the work order into input for failure analysis and plan review.
- Action performed: what was actually done, written for whoever reads the history later.
- Materials used: parts and quantities, tied to the order cost.
- Labor (man-hours): who executed and how many hours, the basis of cost and capacity.
- Start and end date and time: the real execution times, without which there is no repair duration calculation.
- Performer and approval: who did it and who validated the closure, with signature.
Why these fields become an indicator
Each work order field feeds a calculation that maintenance planning does later. Some direct examples:

- Equipment and tag combined with maintenance type show which assets concentrate corrective work, candidates for plan review or replacement.

How to fill in the work order in 6 steps
- Opening: record the work order number, date, requester, equipment with tag and the description of the problem or planned service.
- Classification: set the maintenance type and priority. These two fields decide when the order enters scheduling.
- Planning: before execution, indicate the expected materials and, if needed, the equipment condition (stopped or running) for the service.
- Execution: in the field, record the start time, the identified cause, the action performed, the materials actually used and the end time.
- Labor reporting: log each performer with the hours worked on the order.
- Closure: the performer signs, the person responsible for maintenance planning or supervision validates, and the final status is recorded. A pending item generates a new work order, it does not reopen the old one.
A golden rule for step 4: fill in close to the event. A record made hours later, from memory, rounds off times and loses the cause, and the indicator calculated on it inherits the error.
5 common mistakes when using the work order
Standardizing the form is the beginning. These are the mistakes that most erode the value of the work order once it is in use:
- Closing a work order without a recorded cause. "Fixed" is not a cause. Without the cause field filled in, analyzing repeated failures becomes impossible and the preventive plan never learns from the corrective work.
- Logging times from memory at the end of the shift. Start and end estimated hours later become round numbers, and the repair time calculated on them loses its meaning.
- One work order for several services. Grouping different tasks in the same order mixes times, materials and causes of distinct equipment, and the history per asset gets contaminated.
- Skipping closure approval. Without validation, orders stay open forever or close without verification, and the team real backlog stops being known.
- Treating the description field as bureaucracy. The description written in a hurry today is the information that will be missing for the technician who services the same equipment six months from now.
From paper to digital: when the template is no longer enough
